Please email motherlode@motherlodetrio.com and specify the quantity of CD's to be purchased and shipping address.

We will notify you via email of the total amount due including shipping and handling.

Or you can fill out our order form and fax or mail it to us:

1. Click on the "ORDER CD" button to access an Acrobat PDF file.
2. Fill in the form directly from your computer by mousing over the required information.
3. Print out the completed form using the Acrobat Reader's Print Icon.
4. Mail the completed form and payment to the address indicated on the form.
